Senior Community Care of North Carolina Provides For All Health Care And Social Needs Of Seniors Who Enroll In Our Program.

Services are provided at our PACE Center, at home, or in other locations that match the needs of participants. Our organization does not discriminate in the delivery of services based on race, ethnicity, national origin, religion, sex, sexual orientation, age, mental or physical disabilities or source of payment.

Prospective PACE participants must receive all needed health care services, including primary care and specialist physician services (other than emergency services), from Senior CommUnity Care of NC or an entity authorized by Senior CommUnity Care of NC.

DISCLAIMER: All services are based on need and offered after assessment by the Senior CommUnity Care PACE Interdisciplinary Team. PACE participants may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services.
